You, your GP and your HIV

If you are HIV positive we would very much like to hear your view about visiting your GP. Please take a few moments to fill in this survey. It should take less then ten minutes to complete. We do not ask for any personal information other than your post code if you are happy to give it.

This survey has been compiled by HIV positive patients and it is designed to help us better understand both your relationship with your GP and how you would like to see Primary Care services develop in the future.

If you prefer to leave comments about the service you get from your GP rather than filling out the survey you can add your comments here.
